Is an appointment needed?
Yes, please call our patient coordinator to schedule your visit. We want to ensure we dedicate the appropriate time and care to your needs.
What should I bring to an appointment?
Please bring your insurance information and a valid photo I.D. Having these items ready helps streamline your check-in process.
What if a cancellation or rescheduling is needed?
We understand that things come up. Please call our office ASAP. Less than 24-hour notice could result in a $50.00 fee. We appreciate your understanding.
What if I do not have insurance?
Self-payments are due at the time of service, but please feel free to reach out to our patient coordinator for all of your payment options prior to your visit.
Which insurance plans are accepted?

What instructions do you have for first time visits?
Please come 15 minutes early to fill out paperwork.
Are you contracted with OWCP?
Yes, we are contracted with OWCP.
What if I am late?
To ensure all patients receive the attention they deserve, patients who are more than 15 minutes late will need to reschedule. We value your time and strive to stay on schedule.
When are co-pays due?
At the time of service.
How are telehealth services accessed?
Patients will be seen through the Practice Fusion portal. You will be provided a link through either email or by text. You will use that link to connect to your provider at the time scheduled for your visit. Telehealth makes getting care easy and accessible.
How are medical records or test results accessed?
You may access your records through your Patient Fusion Portal. Please contact our office for instructions and your pass key. Your health information is always at your fingertips.
How is the patient's privacy protected?
Our clinic is HIPAA-compliant, and a copy of our “Patient Privacy Practices” can be viewed on our website or in our office. We take your privacy seriously.
Can health information be shared with family members?
Only upon receipt of a records release waiver, signed by the patient and it must designate the person to whom the records will be released.
